Quit & Get Fit Terms and Conditions

1. Participation in this scheme is open to individuals aged 16 and over who reside
within the Liverpool area.
2. By signing up for the service, clients agree to these terms and conditions. The
scheme offers the opportunity to use a gym pass, which is valid only at Lifestyles
gyms
3. At the 4 week appointment, a gym pass valid for 4 weeks will be offered. This is available
exclusively to clients who have set a quit date and can provide a verified carbon monoxide
(CO) reading. If a smokefree CO-verified/Self-Report quit reading is not provided, a gym pass will not be offered. 
4. At the 8 week appointment, a gym pass valid for 8 weeks will be offered. This is available
exclusively to clients who have set a quit date and can provide a verified carbon monoxide
(CO) reading. If a smokefree CO-verified/Self-Report quit reading is not provided, a gym pass will not be offered.
5. At the 12 week appointment, a gym pass valid for 8 weeks will be offered. This is available
exclusively to clients who have set a quit date and can provide a verified carbon monoxide
(CO) reading. If a smokefree CO-verified/Self-Report quit reading is not provided, a gym pass will not be offered.
6. The gym pass will commence once Lifestyles has contacted the client, and a start date has
been set.
7. Only one 4,8 and 12-week gym pass is available per client.
8. SFL reserves the right to cancel, suspend, or modify the Quit & Get Fit campaign terms
and conditions. The decision of SFK is final and binding, no correspondence will be
entered into regarding this decision.
9. Clients may be invited to take part in social media promotion, though involvement is
entirely optional.
